Bring Insurance Card and picture ID (such as driver's license) when you come to the Surgery Center.

Please be sure to bring any paperwork completed in your physician's office pertaining to surgery which your physician's office instructed you to bring to the Center.

FOR PEDIATRIC PATIENTS, please make sure you have their social security number.

If you have a Living Will, we would like a copy for our records if possible.

IF YOU HAVE POWER OF ATTORNEY OR GUARDIANSHIP of the patient, please bring legal documentation of such.

There will be numerous forms to sign and to be filled out that pertain to your state of health and personal information.

to protect your privacy, the only people that will be allowed in the treatment area are the ones you list on the consent form - which you will fill out in the registration area. Only two people will be allowed in the treatment area at a time.
